1)     Roles of Crisis Team - Immediately following a major fire incident, the Managing Director will activate the Crisis Team to lead and coordinate the efforts:
危機小組的作用-在重大火災后,相關人員會派出危機小組來帶領以及監管下工作
a)     To assess the impact of the incident so as to define the degree, areas, facilities and workforce of damages.
對時間的影響,地點,設施以及損壞進行考察
b)     To bring the immediate situation under control so as limit the effect e.g. intact facilities will not be affected by subsequent actions etc…
立刻控制局面並減少影響,例如確保事件后的任何動作都將不會影響到設施等等
c)     To implement immediate actions to minimise effects.
立刻採取行動以降低其影響程度
d)     To implement medium and long term action plans to restore the services and operations e.g. relocate all facilities in new building, purchase of equipment, etc…
執行中長期的行動計劃,重新讓其運轉,例如將設施移動到新的地點並採購新器材等等
e)     To monitor the process and adjust the action plan.
監控,進行並改善行動計劃

e)     MIS
i)     Carry out damage assessment.
提供損壞報告
ii)     Resuming and implementation MIS systems (e.g. internet, computer networks & infrastructure (e.g. telephone etc…)
重新設置MIS系統(例 網絡,電腦連接以及基礎設施 如 電話
f)     Safety
安全部門/人員
i)     Coordinator for communication with legal department (e.g. safety licenses etc…).

i)     Maintain an updated disaster recovery plan of STR-SH.

h)     Deputy of the Member - Deputy will take up the roles and responsibilities of the member in case of his absence.   Deputies or other personnel from inside or outside STR-SH may be requested to join the team or meeting according to need.

2)   Responsibility of Team Members - The prime responsibilities is defined as below. These may need amendment dependent on personnel availability.

a)     Chairman - Public media director and coordinator for STR resources.

b)     F & A

i)     Coordinator for communication with statutory authorities, insurance, finance and STR employees.

i)     Customer Care and media information (with prior approval by Chairman) coordination.
ii)     Set up a Temporary Help Desk for one week as a nominated telephone station to answer general enquiries from customers and media

d)     Operation & Quality
i)     Coordinator for employee contact/support.

iv)   Coordinator for procurement of replacement laboratory equipment.

3)    Incident Control & Evaluation - Immediately following an incident members of operation & quality, safety and MIS will assess and report on the damage to the Crisis Team.   The crisis team will support and advise emergency services and local municipal units in handling the immediate situation and provide information required about materials and activities of STR-SH.
事件控制和价值评估-在接下来的事件操作和质量,安全和管理信息系统,都会对危机队的损坏进行报告和评估。危机队将维持并建议紧急事件服务商和地区市政单位来着手处理紧急状况并提供有关STR-SH放射及材料所必须的信息。

a)    Based on the assessment, the team may hire contractors to fix damages that may cause another incident e.g. damage of fuel tank, broken windows, bending of water pipe etc…
基于估价金额,组织可能出钱雇人来修理会引起其他事故发生的毁坏处,包括燃料桶的损坏,窗户的破裂,水管的弯曲等…

b)    According to the findings of the assessment, the crisis team can determine immediate actions and do analysis for making up a solid recovery plan.

4)    Damage Assessment - Following the incident, and concurrent with the investigation as to the cause, an assessment will be made

a)    To get information of building safety and availability of building infrastructures e.g. electricity, gas, water, lift, telephone, internet, fire safety system and devices etc…
要得到大厦安全和大厦基础设施即电、气体、水、电梯、电话、互联网、防火安全系统和设备的信息及实用性等...


b)    To determine floor safety and areas (office, storage room etc…) of damage inside STR units in different floors.
确定地板安全和在STR 单位里面 (办公室, 贮藏室等区域..) 损伤程度不同的地板。


c)    To determine number of casualties and the extent of injuries (loss of workforce in short and long periods).
确定人员伤亡的人数和受损范围(包括短期和长期的劳动力损失)

d)    To identify internal installations (e.g. computer network, lighting, electricity, compressed air etc…) & operation equipment (e.g. test equipment, office equipment, storage tanks and supply) affected. (refer to Cecas system)
鉴别内部受到影响的装备(例如计算机网络,照明设备,电器,压缩空气等)和操作设备(例如测试设备,办公设备,贮藏冠及供应品)。(参照非洲经济委员会制度)

e)    To determine the degree of damage of installation & equipment so as to classify them into repair or replacement groups).
确定设施装置的受损程度以便分类他们入修理或替换的小组) 。

5)    Damage Analysis - With the data from the assessment, the crisis team can do analysis to quantify the following effects:-

a)    Testing & inspection capability e.g. number of test delayed per day etc..

b)    Customer service e.g. number of order or calls missed etc…

c)    Employee Loss.

d)    Public image of STR-SH e.g. how many % customer will not use STR-SH.

e)    Financial Loss.

6)    Immediate Actions

Chairman    - Inform STRUS about the incident.
- Call for meetings.
F & A    Inform Insurance agency
Sales    Prepare announcement for customers
Operation & Quality    - Carry out damage assessment.
- Attend welfare needs of any casualties & their families.
MIS    Carry out assessment on MIS system
Safety    Carry out assessment on safety system
Coordination    Assist Chairman to arrange meetings and communication channels to other units (internal or external).

7)    Recovery Plans - In establishing recovery plans, there are 4 possible scenarios from a major fire disaster:-

a)    Total Loss of Office, Laboratories and Storage - This would be a total loss scenario in which local service & operations would not be viable for a significant time period (more than six months).

i)    Chairman: A meeting will be held in the temporary office (e.g. local hotel)   Regular public announcement should be made to keep the STR image.

ii)    F & A: Figures of setting up temporary operations and total resuming should be estimated.   Quick financial supports provided by insurance company and STRUS should be opened.   Welfare needs of employees should be taken care by close contact.   Purchases of office equipment are carried out for the new permanent STR-SH location.

iii)    Inspection: Currently, most of inspection order is performed by China inspectors so STR-SH should setup a temporary coordination & customer services in another building or other offices in Shanghai that can provide communication facilities.  

iv)    Testing: Due to total damage of laboratory equipment, no test operation can be resumed in short time.  Samples can be subcontracted to other local laboratories or our sister laboratory in Shenzhen.   A temporary coordination and customer services office should be set up in another building or other offices in Shanghai that can provide communication facilities.   (CNAS accredited laboratory web link http://www.cnas.org.cn)

v)    Sample Delivery Services: Sample courier service should be switched to TNT headquarters in order to maintain samples collection & delivery services between customers and other laboratories.

vi)    MIS: Remote backup data, programs and documents should be reinstalled in temporary offices so as to resume inquiry, order receiving, communication, result reporting & result posting services etc…  

vii)    Sales: To assist Chairman to prepare updated announcements.   Sales and marketing personnel should contact the affected customers for providing immediate info and helps e.g. resubmission of samples, rescheduling of inspection or delay of test results.   For other customers, sales department should keep an efficient channel for them to place inquiry and order.   To maintain a Help Desk and a Temporary Contact List of Operation.  (see appendix 03 for the announcement template)

viii)    Quality: QA should monitor the temporary operations and resuming works so as to maintain the quality requirements of STR quality system.  Purchases of new laboratory equipment are carried for the new permanent STR-SH location.   Current original certifications e.g. business registration, CNAS, clients’ certificates, safety license and certificate etc… can be replaced by sending requests to the providers. (See appendix 04)

ix)    Storage: Temporary storage areas should be setup in the same building or other nearby buildings.

x)    Drills and tests should be arranged regularly or after critical changes or updates.

b)    Loss of Laboratory – Follow the relevant clauses in section 10)a).

c)    Loss of Storage - Old samples, old test paper records, old account / financial paper records are lost.   Critical contents may be retrieved by contacting the original holders or other copy holders.

d)    Partial Loss of Office, Laboratory and Storage
i)    The impact of a partial loss of either facility is the most likely event and it may be harder than total loss for management since we have to take care of the partial operation and re-build in the same time.

ii)    Repair & replacement of installation, equipment, and infrastructure can follow the relevant clauses in section 10)a).

8)    Important Recovery Task
重要的重建任务
a)   Setting up a new location

i)   Office / Laboratory / Storage

In Pudong district, new locations suitable to our needs should be found by using a
local property agency (see appendix 05). If a single location with area up to 4,000 sq. meter is hard to find, other district should be considered.   For temporary purposes, separate locations can be accepted.
在浦东,寻找新的办公场所应该交由房产代理(?)来做(见附录5)。如果4000平米以上的单一场所难以找到,应该考虑其他的地方。作为临时使用,分开办公应当被接受。

ii)   The infrastructure of the new locations must be assessed to meet our special needs e.g. laboratory need special arrangement for air exhaustion and waste water drainage etc…
   新场所的基础设施应当满足我们的特殊需求,例如:实验室需要特殊的通风设备和污水处理设备
iii)   Decoration and installation time of different operation units are quite different, so a project management team should be formed to oversee the process.
不同的施工单位所花的装修事件差异相当大,因此应当成立一个项目管理小组来监督装修进度

iv)   The estimated time of setup new locations (including installation of old equipment) for different functions are shown below. It is assumed that location design and old equipment are ready for installation

v)   In Shanghai, change of business registration address, registration of other public services like electricity, water, telephone fire and renewal of insurance can be approved within four weeks.

b)   Infrastructure - Temporary and permanent replacement actions are summarised in the table below.

c)    Equipment
i.    Most of the equipment are imported and consequently involve a long lead time for delivery.   Some such as the GC-MS, UV-VIS equipment are available in 1-1.5 months; light fastness tester requires 6 months but most others are 2-3 months.   2nd hand or refurbished equipment can be considered.

ii.    For CNAS accreditation, a period of 4-6 weeks records of stabilisation in the conditioning room would be needed before an audit. The lead time for requesting a visit is about 10-12 weeks.

iii.    Overall, it is estimated that to establish a new operational location would require a good project manager and take 6 months.

iv.    A full list of equipment can be found in Cecas system.

d)    Personnel
i)    A departmental contact list of all department heads & his deputies are maintained by F & A.

ii)    Department head and his deputy should keep a contact list of their staff.

e)    Data, Software and Application - In order to protect MIS system and enable a rapid restart of operation, back-up copies of the following data, functions and applications (software) are maintained in Data Centre.

i)    All e-mail communications go through Data Centre

ii)    Setup all web-mail accounts for all necessary users

iii)    Setup VPN accounts for all necessary users

iv)    Re-direct all applications to Data Centre

v)    Re-direct company’s web site to Data Centre

f)    Documentation - Controlled copies of this document will be held by the personnel in the crisis team including their deputy.   A copy will be kept by STRUS (Chairman of Disaster Recovery Team).

i)    Additions and amendments to this manual will be made as required to ensure that the information is kept up-to-date.

ii)    DR Coordinator is responsible for the maintenance so all members of the crisis team have a current copy on hand.
