单面吸塑加纸卡 Single side blister with paper board
(usually the blister is having a groove for card to slide in or it is welded on to the card)
工业白盒加泡沫内衬 White box with foam liner
(dont understand what is industrial... you may use printed (specified # of colors) white box..

I thought "blister" is "泡罩“, like the packaging used for a lot medicine pills, which is different from "吸塑", but again I know little about this kind of stuff.
